Tailored Care Pathways for Neurological Conditions

We offer a range of specialist care services to support individuals with complex neurological conditions at every stage of their journey. Explore our different care pathways to find out more about how we provide tailored, multidisciplinary support.

Complex Disability Management

We provide medium and long-term specialist nursing care for people with complex or profound neurological disabilities requiring high levels of clinical support. Our experienced multidisciplinary team delivers advanced care tailored to the specific needs of each resident, including complex wound management, enteral nutrition, and postural management to optimise comfort and prevent secondary complications. We also implement structured management plans for cognitive and behavioural impairments, ensuring that both physical and psychosocial needs are met. Maintenance physiotherapy, provision of assistive technology, and ongoing therapeutic interventions support mobility, communication, and engagement in daily activities.

Our team works closely with each resident to develop tailored care plans that promote autonomy, enhance quality of life, and encourage participation in meaningful activities. Whether supporting an individual to engage with assistive technology, facilitating community access, or providing one-to-one engagement such as reading or sensory stimulation, we ensure that care extends beyond medical management.

Rehabilitation

At Dee View Court, we provide specialist, goal-oriented rehabilitation for individuals with complex neurological conditions, supporting them to optimise function, enhance independence, and improve overall quality of life. Our multidisciplinary team, which includes physiotherapists, occupational therapists, and recreational therapists, works closely with each resident to develop personalised therapy programmes that support their specific goals and capabilities. Rehabilitation may focus on maintaining or improving mobility, relearning essential skills, or adopting new strategies to navigate daily activities with greater confidence. For some individuals, this support enables them to transition to less intensive levels of care or regain the ability to live independently.

Therapeutic interventions are tailored to each individual’s needs, whether that involves strengthening exercises, neurophysiological techniques, adaptive equipment, or communication support. Our structured programmes are designed to maximise potential, using evidence-based rehabilitation techniques to promote recovery and long-term functional gains. By taking a holistic approach that considers both physical and cognitive rehabilitation, we empower individuals to regain control over their daily lives, adapt to new challenges, and work towards greater independence in a safe and structured environment.
