We work closely with Health & Social Care Partners to ensure a smooth transition into our service, with comprehensive assessments and care planning tailored to each individual’s clinical, functional, and psychosocial needs.
Our approach integrates 24-hour specialist nursing care with physiotherapy, occupational therapy, and recreational therapy to optimise function, mobility, and quality of life.
